Access the Consent Forms Section
Log into your Keep The Fees account.
In the left sidebar, click Forms.
Click Consent.
Click the Create Form button.
Basic Form Settings
Enter the Form Title (e.g., “Tattoo Consent Form”, “Piercing Consent Form”, or “Minor Consent Form”).
(Optional) Enter a Redirect URL — clients will be sent here after completing the form (Instagram, Google Business page, etc.).
(Optional) Enter your Google Analytics ID to track form usage.
Select the Form Language (English or Spanish). Note: Language only changes navigation and labels. All custom fields must be written in the selected language.
Assignment and Category Settings
In Auto Assign To, select a specific artist if desired (optional for general forms).
Select a Walk-in Category — the minimum age set for that category will apply to this consent form.
Form Link and Customization
Copy the Form Link to share with clients.
Generate and download a QR Code for the form (recommended to print and place at the front desk).
(Optional) Create a custom short URL by clicking Settings and adding a unique ending (e.g., “/sparrow”).
Consent Form Text
Enter the general disclosure text that appears at the end of the form. Make sure it includes all required legal information for your state and county.
Additional Options
Show Preferred Artists (highly recommended) — clients will see a dropdown to select their artist.
Notify Artists Upon Submission — the selected artist will receive an email, SMS, or push notification.
Build the Form Fields
Basic fields (name, date of birth, email, phone, address, gender, photo ID, and signature) are automatically included. You only need to add custom fields.
Add a New Field
Click the plus (+) button to open the field menu.
Field Options (for each custom field)
Required — client must answer before submitting.
Editable — allows editing later (useful for ink lot numbers).
Prefill — automatically fills from previous forms (turn off if your state requires fresh information each visit).
Internal — field is visible only to staff, not the client.
Required to Checkout — form cannot be completed until this field is filled.
Available Input Types
Checkbox — For confirmation statements (e.g., “I understand I am getting a tattoo by a licensed artist”).
Text Field — Short answers (e.g., ink lot numbers).
Large Text Field — Longer answers (e.g., “What allergies do you have?”).
Yes / No — Simple yes/no questions.
Yes / No / Other — Yes/no with space for additional explanation.
Dropdown — Single selection from a list (e.g., “What service are you getting?”).
Display Only Text — Static instructions or section headers.
Image Upload — Allows clients to upload photos (e.g., reference images, old tattoos, or required documents).
Drag and drop fields to reorder them.
Enable or disable fields as needed.
Click Save when the form is complete.
Best Practices
Use clear, descriptive titles for each consent form.
Make critical fields Required and Required to Checkout where appropriate.
Use Internal + Editable for ink lot numbers or staff-only information.
Enable Show Preferred Artists and artist notifications for better workflow.
Print the QR code and keep it at the front desk for easy client access.
Always include a complete legal disclosure in the consent form text.
