Log in to your Squarespace account and navigate to the page where you want to embed the form (e.g., under the "Fill Out a Request" section).
Click Add Block on the page editor.
Select the Embed block from the list.
In the embed block, click the Pencil Icon to edit.
Choose Code Snippet (instead of the default embed options).
Under Embed Data, paste the following embed code:

Click "Back" (or close the editor) to apply the code. The form preview will appear in the block.
Adjust the size as needed:
Drag the block handles to make the embed wider or taller.
Change the height or width parameter in the embed code (e.g., height="800").
Preview the page on desktop and mobile to ensure the form displays correctly.
Click "Save" and exit the page editor.
That's it! Your form is now integrated into your site, helping to optimize workflows and deliver an elevated experience for your clients.
